In this issue, the featured innovations and QualityTools address the complex topic of managing chronic health conditions. An estimated 133 million people — or about one in two Americans — live with a chronic condition such as diabetes, asthma, high blood pressure, or depression. Of that number, about half experience multiple chronic conditions.

Given that high prevalence, health care organizations are challenged to provide coordinated, efficient, and effective health care to patients with chronic conditions. The featured innovations explain how three health care organizations addressed these challenges.

Innovation Profiles:

- E-Mail and Telephone Contact Replaces Most Patient Visits in Primary Care Practice, Leads to More Engaged Patients and Time Savings for Physicians
- Employer-Based Telephone Screening, Outreach, and Care Management for Depressed Workers Improves Clinical and Workplace Outcomes
- Group Primary Care Visits Improve Outcomes for Patients With Chronic Conditions
- Multifaceted Program Helps Pediatricians Screen for Maternal Depression and Assess Infant Crying and Toilet Training, Enhancing Their Ability to Prevent, Identify, and Address Cases of Potential Child Abuse
- Remote Retinal Screening Facilitates Diagnosis and Treatment of Retinopathy for Poor and/or Uninsured Patients With Diabetes in Rural California
- Surgical Hospitalist Model Enhances Access to Surgical Consultations, Increases Physician Perceptions of Quality at Academic Medical Center
