Learning Networks
The AHRQ Health Care Innovations Exchange provides several ways for users with similar
interests to connect with each other through learning networks, often called communities of practice.
Through these learning networks, you will be able to exchange ideas, experiences, and information about
specific topics; learn what works and what doesn't from peers; and work together to solve common problems.

Coordination of Community Care for At-Risk Populations
There are several models for coordinating community-level care for populations with complex
health and social service needs. While there are distinct differences within each of these
types of models, a common thread is their focus on connecting patients or consumers to one or
more needed services or supports. This learning network will begin by focusing on the Pathways Model.
Participants will learn about the unique features of this model and how it is being implemented,
exchange information about lessons learned, and explore solutions that help improve patient outcomes.
