Discharge Risk Assessment


This risk assessment tool, to be completed 2 days before a hospital patient is discharged, can help discharge planners better understand the needs of the patient, thereby making readmission less likely. It provides clear guidance for referring patients to home health or hospice care. Case managers use the simple risk factor checklist to calculate a patient’s risk score and make recommendations to home health or hospice services based on the patient’s score and the eligibility criteria listed on the tool.
