Guide to Long Term Care for Veterans


Description

This guide provides tools and resources for veterans, caregivers and families, and U.S. Department of Veterans Affairs (VA) providers and staff to explore long-term care options. The guide includes information about home- and community-based services, residential settings, and nursing homes.

It also provides a shared decisionmaking approach to long-term care decisions in which veterans work with their social worker, care team, and informal caregivers to make plans and decisions about long-term care. Veterans can use the Shared Decision Making Worksheet to help them think about long-term care services and settings that best meet their needs and preferences. They can ask their caregiver or support person to complete the Caregiver Self-Assessment, which helps their caregiver review their roles, responsibilities, and stress level.

The site also features personal stories and links to other resources on long-term care.
