Community Health Workers Toolkit


The Community Health Workers Toolkit is designed to help rural communities evaluate opportunities for developing a community health worker program, and to provide resources and best practices developed by successful community health worker programs. Through eight modules, the toolkit offers tools and resources to develop and implement programs for using community health workers in local programs.
