The Diabetes Wellness Center Web site, developed by the New York City Health and Hospitals Corporation (HHC), is an online resource for people who have been diagnosed with diabetes. The site provides tools and tips to help diabetes patients become active partners in their care management.

  • Healthy eating and exercise tips
  • Access to free blood monitors
  • Downloadable forms to help patients monitor their condition

The Web site also offers information about support groups and diabetes wellness classes available in the health system's 11 acute care public hospitals.

Users can sign up to get e-mail alerts about diabetes tips, free offers, new recipes, and advice from diabetes experts.
