Get Connected! Toolkit: Linking Older Adults With Medication, Alcohol, and Mental Health Resources


Description

The Get Connected! Toolkit was created to help aging service providers learn more about alcohol and medication misuse, and mental health problems in older adults to address these issues more effectively. It has been designed to help aging service providers undertake health promotion, advance prevention messages and education, and undertake screening and referral for mental health problems and the misuse of alcohol and medications. This toolkit helps coordinate these efforts and links organizations and the older adults they serve to other valuable community-based and national resources.
