This toolkit offers resources to help physicians/providers and their staffs address communication challenges that may result from patients’ literacy levels or language/cultural barriers.

The toolkit contains the following materials:

• Information on health literacy, advance directives, palliative care, and cultural competency, with tips that physicians/providers and their staffs can use to improve communication with their patients for better health outcomes
• Forms to document the end-of-life discussion, durable power of attorney for health care, and a living will
