The Assistance for Clinical and Community Teams in Improving Outpatient Health Needs (ACCTION) Pack is a Web portal to a wealth of information about guiding principles, case studies, and prior research for practical, collaborative approaches to prevent and manage major chronic illnesses. It includes tools to guide delivery system change to prevent and manage major chronic illnesses. Health practitioners and community professionals can use the tools to develop partnerships through evidence-based assessment and intervention strategies. 

The ACCTION Pack addresses:

  • Three cardinal risk factors (physical activity, nutrition, and tobacco use)
  • Three common chronic diseases (obesity, asthma, and diabetes)

The site includes:

  • Background information about each condition
  • Needs and resource assessment of chronic disease services for practices
  • Searchable toolkit of resources to help improve these services
  • General guidelines for implementation strategies
