square iconDescription

This rating scale for nurses and other health care providers predicts a patient’s level of risk for developing pressure ulcers. The six subscales include:

  • Sensory perception
  • Moisture
  • Activity
  • Mobility
  • Nutrition
  • Friction and shear
Permission is required to use the forms. To receive permission, users are required to share personally identifiable information.
