Dysphagia Screening Assessment Tool


The Dysphagia Screening Assessment Tool is designed to be used by nurses prior to any oral intake to screen patients presenting with stroke-like symptoms or any patient in whom dysphagia is suspected. It includes a list of warning signs for possible dysphagia as well as the developing hospital's policy and procedure for using the tool.
